Application & Release Form

If you are applying to Villa Duchesne and Oak Hill School, please download and complete our Enrollment Application Form as well as our Student Information Release Form and submit with a $50 application fee.

If you are applying to multiple independent schools for grades 5-12, the Standard Application Online (SSOA) is common to most members of Independent Schools of St. Louis. A $50 application fee does apply. After completing the SSOA application, please print the release form along with either of the teacher recommendation forms when prompted and deliver them directly to your student's school. We require at least one recommendation from a student's academic teacher. We will accept additional recommendations if the family chooses to submit them.
